Tips for Completing Form I-134
1. Question 3. Enter information about your fiancé. If your fiancé has children who will be coming with her make sure to their names and information under the subsection titled “Name of spouse and children accompanying or following to join person.”
2. Question 7. Enter the your occupation or type of business or industry you work in the field titled “Type of Business.” Enter your company name or place of employment in the field titled “Name of Concern.” Enter the address of your business or place of employment in the remaining fields.
Enter your annual income as stated on your most current tax return in the first line below your employment information.
Enter the total amount of money you have in all of your checking or savings accounts on the second line. (*This should include certificates of deposits or CDs.)
In the third line enter the value of any other personal property you have such as cars, RVs, boats, etc. If you don’t have any other personal property of significant value enter “N/A.”
In the 4th line enter the value of any stocks, bonds or other securities you own. This includes any other financial assets you have such as 401K accounts or other investment accounts.
In the 5th line enter the amount of any life insurance policies you have. If you don’t have any life insurance enter “N/A.”
In the 6th line enter the cash surrender value of any life insurance policies you own, if any. (*Some life insurance policies do not have cash value. Contact your insurance agent to assist you with this.) If you don’t have life insurance or the insurance you have has not cash surrender value enter “N/A.”
In the 7th line enter the value of any real estate you own. (*You can use the value of a recent appraisal. If you haven’t had a recent appraisal you should be able to come up with a reasonable estimate based on the value of similar properties in your area. You do not need to have a formal appraisal but use a conservative number.)
In the line immediately below this enter the current amount owing on any mortgages against the property.
In the last line, enter the address of the property you own. (*If you own more than one property attached a separate sheet indicated the address, value, and current balance on any mortgages. Write “See attachment in this space.”
3. Question 8. Enter the names of any persons who are dependent on you for financial support. Also, indicate if they are wholly or partially dependent on you. (*You should include any person who you list as a dependent on your federal tax returns here.)
4. Question 9. Enter the names of any persons for whom you have completed this form before. If you haven’t submitted any petitions before write “None.”
5. Question 10. Enter the names of any persons for whom you have submitted a visa petition to the USCIS before (excluding your fiancé in this petition.) If you haven’t submitted any petitions before write “None.”
6. Question 11. Check the box next to “Intend” indicating that you intend to provide financial support for your fiancé. (*Even if your fiancé may eventually be able to financially support herself you should check this box.) In the space below write “I will provide any financial contribution to support my fiancé.”
7. Oath and Signature. Read the Oath and sign and date the form in the presence of a Notary Public. THIS FORM MUST BE NOTARIZED.
