Form G-325A
You must complete two sets of Form G-325A: one for yourself and one for your fiance. Form G-325A asks for additional biographical information. Each form G-325A is completed in quadruplicate (4 copies.) However, you only need to enter the date on the first page and the other 3 pages will be auto-populated.
You will need to submit this form again later in the process when you apply to adjust your fiance's status to that of a lawful permanent resident. We recommend that you save this if possible or make extra copies for use later so you do not have to complete this form twice.
