USCIS issues revised form I-693

The USCIS has issued a revised Form I-693 Report of Medical Examination and Vaccination. The new form is dated June 5, 2008. The previous edition will be accepted through July 14, 2008.

Form I-693 is most commonly used in association with a petition for adjustment of status. The purpose of the form and the examination is to establish that the prospective immigrant does not have a medical condition that would be a ground for exclusion from the U.S.
